Complication insurance compared
The full detailed analysis of the current complication insurance tariffs (Folgekostentarife) – beautyprotect LITE, PRO and VIP, medassure beauty and safe4beauty. Prepared and reviewed by Gehling Consulting – independent actuaries (Versicherungsmathematiker, Aktuare) – on behalf of quadmed AG, based on the official policy terms and conditions.
This is a translation of the German original. In case of any discrepancy, the German version of this comparison and the insurers' own policy terms and conditions (AVB) are authoritative. Insurance terminology is jurisdiction-specific; German legal terms are given in brackets where an exact English equivalent does not exist.
What does complication insurance cover?
Complication insurance (Folgekostenversicherung) covers treatment costs arising from complications after cosmetic surgery – for example after a breast augmentation, liposuction or rhinoplasty. The most important building block: statutory health insurers are permitted to reclaim treatment costs from the patient in the event of complications following aesthetic procedures (recourse claim (Regress) under § 52 SGB V) – complication insurance assumes this claim up to EUR 300,000, and with beautyprotect VIP up to EUR 500,000. Higher-tier tariffs additionally pay for private follow-up treatment by the surgeon who originally operated or in a private clinic, on both an outpatient and an inpatient basis. Individual tariffs even cover corrections that are not medically necessary, for example in the case of objective aesthetic impairments such as raised scars or displaced implants. By contrast, purely subjective dissatisfaction with the result is not insured with almost all providers. Table 1 shows what the individual tariffs provide in detail.
1 Overview
Complication insurance covers treatment costs that are medically necessary as a result of complications following cosmetic surgery or another aesthetic procedure. Depending on the insurance status – covered by GKV, holding substitutive PKV cover, or entitled to civil servants' health subsidy (Beihilfe) – costs of differing amounts are not assumed by the health insurance provider. The basis for this consists of statutory provisions in the GKV (SGB V § 52), standardised rules on Beihilfe in the various Beihilfe regulations, and the individual agreements in private health insurance.
Even where the medically necessary treatment can be carried out within regular healthcare provision, many patients prefer to be treated further by the doctor who originally treated them. However, that doctor as a rule invoices on a private basis only. The higher-tier tariffs of complication insurance policies also cover this private treatment, whether outpatient or inpatient.
In addition, treatment costs may arise if the result does not turn out as would be subjectively or objectively desirable and corrective treatment is to be carried out. This service, which is not medically necessary, is only offered on a purely private basis outside general healthcare provision. Complication insurance policies in some cases also assume the costs of these non-necessary corrective treatments, e.g. after accidents or if objectively significant aesthetic impairments are present.
This comparison of complication insurance policies describes the tariffs currently on offer — June 2026. The focus of the tariffs is on classic cosmetic surgery in the narrower sense, such as breast augmentation with an implant. Aesthetic treatments such as wrinkle tightening, Botox treatments and so on can in some cases be covered by separate tariffs, even though they are covered by the complication insurance policies considered here. The latter tariffs for "small" treatments are not considered further here.
Section 2 assesses the strength of the benefits offered by the tariffs and summarises this in a rating. Section 3 contains detailed information on the tariffs in tabular form.
1.1 Selection of tariffs
Since the last publication of the comparison in spring 2023, the complication insurance policies on offer have developed further. With "medassure beauty", distributed by the managing general agent (Assekuradeur) JHC Cologne and underwritten by Nürnberger Krankenversicherung, a satisfaction guarantee can be taken out again from 2024, although as a supplementary accident insurance policy to be taken out separately with Dialog Versicherung AG. In addition to the two familiar tariffs, Squarelife Insurance AG has launched a further tariff, "beautyprotect VIP", with higher sums insured. "safe4beauty" has been offered since 2025 by the insurer W. R. Berkley Europe AG – Niederlassung für Deutschland, and in the course of the change of insurer the policy was revised in many details. All the insurance policies are offered mainly through specialised brokers, which have remained unchanged for years. The tariffs considered are: "beautyprotect LITE", "beautyprotect PRO" and "beautyprotect VIP" from Squarelife Insurance AG, "medassure beauty" from Nürnberger Krankenversicherung AG together with "medassure beHappy" from Dialog Versicherung AG, and "safe4beauty" from W. R. Berkley Europe AG – Niederlassung für Deutschland.
The quality of the tariffs has improved overall; the associated documents — AVB and IPID — are now also predominantly easy to understand.
The data status is June 2026.
1.2 Data basis
The comparison of complication insurance policies is based on the contractual agreements as set out in the general terms and conditions (Allgemeine Versicherungsbedingungen, AVB) and further information material such as the insurance product information document (IPID). In individual cases, additional information material from the providers' websites is used, e.g. on the level of the maximum sum insured or the possible policy terms, or the details given in the insurance policy documents (Versicherungsscheine).
1.3 Structure of the comparison
The tariffs are described in the five areas "product design", "scope of cover", "exclusions", "policyholder duties (Obliegenheiten)" and "further AVB provisions". "Product design" describes the type of insurance and key framework conditions such as the policy term, maximum entry age, number of insured procedures and the geographical scope of validity. "Scope of cover" contains the central promise of cover, i.e. up to what amount and in which cases the complication insurance pays. Exclusions and policyholder duties are classic risk-limiting provisions on the part of the insurers.
Because of the complexity of the subject matter, a presentation that is both complete and compact is unfortunately not possible. In order to provide an overview, Table 1 therefore contains a summary of the most important aspects of the tariffs. To improve clarity, the tariffs "beautyprotect VIP" and "medassure beHappy" are not presented separately. On the one hand, because "beautyprotect VIP" is identical in cover to "beautyprotect PRO" apart from the sums insured. On the other hand, because "medassure beHappy" offers only a small additional cover in the event of subjective dissatisfaction, in the guise of a supplementary accident insurance policy.
This overview is suitable for quick orientation. In addition, Table 3 contains further details on the range of benefits, the exclusions, policyholder duties and other aspects. This table provides a comprehensive overview and also addresses points of detail.
1.4 Exclusions in detail
As different as the tariffs are in what they pay, so different are their exclusions – often more decisive for the choice of tariff than the sum insured. All providers exclude subjective dissatisfaction with the surgical result. Squarelife (beautyprotect) works with few, abstractly worded exclusions such as pre-existing conditions or third-party liability (Dritthaftung). medassure beauty excludes, among other things, capsular contracture (Kapselfibrose) grades 1 and 2, Breast Illness Syndrome and treatments outside the countries named in the insurance policy document. safe4beauty has the longest list: in addition to capsular contracture grades 1 and 2, also scars, sensory disturbances, numbness, hair loss with facelifts and blindness with eyelid lifts. Important for women with breast implants: only beautyprotect PRO/VIP insures capsular contracture of all Baker grades. The complete exclusion lists of all four tariffs can be found in Table 1 and Table 3.

*: beautyprotect PRO / VIP — explanation of the case-based flat-rate amount: For private invoices under the GOÄ and for treatment in private clinics, Squarelife Insurance AG provides for treatment costs to be covered by means of case-based flat-rate amounts; see also Annex C: Schedule: Catalogue of private medical services in the beautyprotect PRO / VIP AVB. These flat-rate amounts simplify billing for treatments due to a complication following cosmetic surgery. The level of the flat-rate amounts was derived on the basis of Squarelife Insurance AG's extensive experience of more than 12,000 claims. The flat-rate amounts range from EUR 250 for the simplest treatments up to EUR 5,000 for complex treatments. Multiple payments of the treatment flat rate are also made if several treatments are required, up to the agreed sum insured of EUR 30,000 or EUR 50,000. The most frequent complications are named explicitly. Further possible procedures are captured, for example, via the duration of a treatment, the necessity of a general anaesthetic, or the length of an inpatient stay. Clinics and doctors can either bill the flat rate directly or prepare a classic itemised statement under the scale of fees for doctors (Gebührenordnung für Ärzte, GOÄ), which is then, however, settled only up to the listed flat rate. On the basis of experience, the flat-rate amounts are set such that reasonable invoices are generously covered, while excessive invoices remain outside the scope. The flat-rate amounts are regularly reviewed for appropriateness and are also increased for existing customers. It makes sense to agree these flat-rate amounts with the treating doctor in advance.
An aesthetic impairment exists, for example, in the case of raised scars, double bubble, rotation, dislocation or bottoming out of an implant.
beautyprotect VIP: the sum insured in standard statutory care is EUR 500,000 instead of EUR 300,000, and for private clinics and private practices EUR 50,000 instead of EUR 30,000. The case-based flat-rate amounts are doubled compared with "beautyprotect PRO".
**: medassure beauty: The agreed benefits of "medassure beauty" are documented in the insurance policy document and are not contained in the body of terms and conditions. The details given here are taken from the IPID and the website, in particular the limitation of benefits to EUR 5,000 if the doctor / clinic is changed (website / FAQ accessed on 5 June 2026). According to the IPID, benefit catalogues are also possible with "medassure beauty". "medassure beauty" also pays in the event of complications resulting from dental treatment and dental prostheses.

2 Assessment of the complication insurance tariffs
The decisive factors for the strength of a tariff's benefits are the scope and limits of the insurance cover as well as the level of the sums insured. The area "scope of cover" therefore carries the greatest weight in the overall assessment, at 50 %.
Also relevant are aspects such as cover for past procedures as well, the insurance duration — that is, the period over which complication costs following an aesthetic procedure are assumed —, possible benefit extensions, and the geographical reach of the cover. This "product design" (insurance duration, freely chosen point of conclusion, several procedures, direct linkage to a planned procedure, countries of a procedure or of the follow-up treatment) is weighted at 20 %.
Exclusions account for 20 % of the overall assessment, and the policyholder duties and further provisions 5 % each. Policyholder duties such as a mandatory cost estimate, which have a strong influence on the insurance cover, are taken into account in the area "scope of cover".
2.1 Notes on the individual rating criteria
2.1.1 Product design
Squarelife Insurance AG has revised the existing tariffs "beautyprotect LITE" and "beautyprotect PRO" in certain respects and designed the new tariff "beautyprotect VIP" with significantly higher sums insured and case-based flat-rate amounts, but otherwise identically. Key features such as an ongoing supplementary health insurance policy with monthly automatic renewal are still included. This approach considerably simplifies dealing with complication insurance, especially as aesthetic procedures already carried out before the contract was concluded also fall under the cover. Insured persons do not have to worry about whether planned procedures are covered or whether complications only occur after one or more years. There are also few geographical restrictions.
Nürnberger Krankenversicherung with "medassure beauty" and W. R. Berkley Europe AG – Niederlassung für Deutschland with "safe4beauty" offer a classic complication insurance policy that is tied to one or more specific procedures with a firmly agreed insurance duration. For example, the type of procedure and also the treating doctor or the clinic are asked for in the insurance application. According to its website, "medassure beauty" offers generous insurance durations of up to five years. In addition, cosmetic surgery and complication treatment can, by agreement, also take place abroad. "safe4beauty" has newly included the possibility for the cosmetic surgery to be carried out in selected foreign countries. Any complication treatment must then take place in Germany or in the country of the cosmetic surgery.
The approaches of the three providers differ in this respect. The "beautyprotect" tariffs generally cover the complication costs of further possible procedures as well, without the type of procedure or the surgeon being known at that point. The other providers offer cover for precisely defined operations and request the necessary details in the insurance application. This information can then feed into the underwriting and also into the pricing.
There are also relevant differences in the period within which follow-up treatment after a complication must be started and completed.
2.1.2 Scope of cover
Of the tariffs considered, "beautyprotect LITE" from Squarelife Insurance AG is the only one that does not assume private medical costs outside standard statutory care. The cover is essentially limited to possible GKV recourse claims as well as costs not assumed under PKV or Beihilfe. In this respect this tariff is genuinely a "lite" tariff and cannot be compared with the other tariffs. For this reason, no assessment is made for "beautyprotect LITE" in the category of insurance cover.
In all the other tariffs, private medical costs for medically necessary follow-up treatments are assumed on an outpatient or inpatient basis, in some cases after the regular health insurance cover has paid first. This is the central benefit module. Differences exist in the sum insured and hence in the level of cost assumption for a private doctor's invoice or clinic invoice, and in the billing arrangements. Squarelife Insurance AG has introduced a system of flat-rate amounts for private treatments, which can be billed directly by the doctors or clinics taking account of the scale of fees for doctors (GOÄ). Flat-rate billing simplifies and speeds up the billing process. With its increased sums insured, "beautyprotect VIP" is aimed at specialised and demanding doctors and clinics that charge significantly higher fees than is normally the case.
"medassure beauty" limits the sums insured to EUR 5,000 if the doctor or the clinic for the complication treatment is changed, compared with the usual sum insured of EUR 10,000. This possibly reflects the approach already mentioned, namely that a specific treatment with a specific doctor is insured. Cost reimbursement in outpatient standard statutory care is also limited to EUR 10,000 for private patients and persons entitled to Beihilfe.
For serious complications, the patient's existing insurance cover typically takes effect first, and the complication insurance policies then assume, on a subsidiary basis, costs of a clinic that have not been covered. In these cases, high sums insured of EUR 300,000 are generally customary, and with "beautyprotect PRO" even EUR 500,000.
When it comes to assuming treatments that are not medically necessary, Squarelife takes its own customer-friendly approach and also insures objective aesthetic impairments. These include, for example, raised scars, double bubble, rotation, dislocation or bottoming out of an implant. Even if other providers were to be generous in individual cases where treatments are not medically necessary, the explicit contractual provision at Squarelife creates considerably greater legal certainty for insured persons and reduces the potential for conflict.
With all providers, any consequences of accidents are insured to the extent that, if medically necessary treatment is required, the "restoration of the original surgical result" is also covered. With "medassure beauty" and "safe4beauty" these are included benefit modules; with the "beautyprotect" tariffs this already forms part of the core promise.
"Filler treatments" and the like are regulated fairly extensively and separately by W. R. Berkley Europe with "safe4beauty".
"beautyprotect PRO" and "VIP" newly include a benefit for "breast reconstruction after breast cancer". In the case of breast cancer, breast reconstructions are co-insured which are not medically necessary and are therefore not assumed by the health insurer, e.g. for the second, unaffected breast. This cover is, however, subject to a waiting period of two years after the contract is concluded.
Before assuming the costs of a complication treatment, "safe4beauty" from W. R. Berkley Europe is the only tariff to require a cost estimate for treatment in a private clinic or private practice, which must be approved. Through this approval requirement, the insurer has, with this tariff, a means of intervening in the costs of a follow-up treatment. Unfortunately, no further details on how a cost estimate is handled can be found. Both "medassure beauty" and "safe4beauty" generally limit cost reimbursement in private billing to 3.5 times the GOÄ multiplier factor (Steigerungsfaktor).
2.1.3 Exclusions
The tariffs of Squarelife Insurance AG ("beautyprotect LITE", "PRO" and "VIP") as well as "medassure beauty" from Nürnberger Krankenversicherung work with a comparatively manageable catalogue of exclusions. Squarelife lists exclusions that are worded rather abstractly; Nürnberger Krankenversicherung with "medassure beauty" lists few specific medical exclusions or some general circumstances. "safe4beauty" from W. R. Berkley Europe, by contrast, lists a considerably more extensive series of possible complications that are excluded from cover. For filler treatments there are again separate additional exclusions.
Squarelife insures capsular contracture irrespective of the Baker grade; "medassure beauty" and "safe4beauty" exclude Baker grades 1 and 2. Until now, a biopsy has been required as evidence of capsular contracture. "safe4beauty" explicitly assumes the examination costs of a biopsy and newly also permits evidence of capsular contracture by means of a mammogram. "safe4beauty" also limits the costs of an implant where a warranty claim actually exists.
2.1.4 Policyholder duties
For a valid claim to benefits, the insured person must fulfil the agreed policyholder duties. In addition to the usual and familiar duties such as correct statements in the application, complication insurance policies in particular include duties to inform the insurers in the event of a claim. In general, an insured event must be notified to the insurer without undue delay. The already mentioned assertion of warranty claims against implant manufacturers at W. R. Berkley Europe is found indirectly in the exclusions of third-party liability in the other tariffs. With the obligation under "safe4beauty" from W. R. Berkley Europe to submit a cost estimate and have it approved, this insurer stands alone.
2.1.5 Further provisions
"safe4beauty" permits premium payment after the cosmetic surgery, provided the contract was concluded in good time beforehand. "medassure beauty" can be supplemented by "medassure beHappy" with a small benefit in the event of subjective dissatisfaction.
2.2 Advantages and disadvantages of the individual tariffs
The following points count positively in the benefits assessment:
- extensive insurance cover: GKV recourse, benefits in standard statutory care, private medical billing in a private practice or private clinic, cover for treatments that are not medically necessary
- high sums insured for private medical billing
- conditions for benefits and policyholder duties limited to important and customary aspects
- long insurance durations and cover for past operations
- treatment by the head physician and single/twin rooms in hospital (optional hospital services)
- few exclusions
- cosmetic surgery and complication treatments also possible outside Germany
The following are correspondingly counted as disadvantages:
- low sums insured for private medical billing
- written cost estimate
- wide-ranging exclusions of medical side effects
- restriction on the reimbursement of medication costs
- cover only for medically necessary treatments
- restrictive provisions for treatments abroad
- short periods for carrying out a complication treatment

2.3 Overall assessment
In the overall assessment, the tariffs "beautyprotect PRO" and "beautyprotect VIP" from Squarelife Insurance AG achieve the highest scores. The decisive factors are the assumption of private medical services up to EUR 30,000 with "beautyprotect PRO" and EUR 50,000 with "beautyprotect VIP", as well as the co-insurance of objective aesthetic impairments, including where these are not medically necessary. In addition, the insurance approach with annual renewal without a specific tie to an aesthetic procedure stands out positively. "beautyprotect VIP" also makes it possible to cover exclusive costs.
"medassure beauty" generally pays up to EUR 10,000 for outpatient treatment and a private clinic, and where the doctor or clinic is changed only up to EUR 5,000, but for treatments in public hospitals up to EUR 300,000 for private patients and Beihilfe. Also striking is the limitation of the reimbursement of medication and prescription charges to EUR 400. The integration of the "Unfall-Schutz" and "Ästhetik Plus" modules upgrades the tariff, since these would be treatments that are not medically necessary.
"safe4beauty" reimburses up to EUR 300,000 in standard statutory care. For private doctors and private clinics, however, a cost estimate is generally required, which has to be approved except in emergencies. How this is handled operationally is unclear. In addition, "safe4beauty" also pays for an operation that carries out the restoration of the original surgical result, and thus takes a step in the direction of treatments that are not medically necessary. According to the website, the consequences of an accident are also covered.
Among the exclusions, "safe4beauty" from W. R. Berkley Europe stands out with a comparatively very long list that names many specific medical complications. "medassure beauty" lists considerably fewer points, with only a few specific complications remaining. Squarelife Insurance AG restricts itself to rather abstract exclusions, whereby an exclusion of "third-party liability" is likely to arise most frequently. In addition, changes in shape and dents are explicitly excluded.
The further provisions differ only slightly. Worth mentioning is the generous provision on premium payment up to a point in time after the operation with "safe4beauty".
The overall assessment of "beautyprotect PRO" thus reaches four stars (4.0 points), as does "beautyprotect VIP" with four stars (4.4 points) on a scale of one to five stars. "medassure beauty" achieves 3.1 points and therefore three stars, as does "safe4beauty" with 2.7 points and three stars. "beautyprotect LITE" receives no overall assessment because its benefits are restricted to standard statutory care, even though the other provisions are comparably good to those in the "beautyprotect PRO" tariff.
beautyprotect vs. medassure – the direct comparison
The two most sought-after providers in direct comparison: beautyprotect (Squarelife) wins the 2026 rating with 4.0 (PRO) and 4.4 (VIP) out of 5 stars; medassure beauty achieves 3.1. In favour of beautyprotect are the higher sums insured for private treatments (EUR 30,000/50,000 instead of EUR 10,000), cover for procedures already carried out, the open-ended term with monthly cancellability, and cover for objective aesthetic impairments including capsular contracture of all grades. In favour of medassure are the assumption of acute hospital costs up to EUR 300,000 for private patients and the cover that can be extended by option ("beHappy" in the event of subjective dissatisfaction). Points to note with medassure: only EUR 5,000 if the doctor or clinic is changed, medication only up to EUR 400, a fixed contract per procedure. The actuaries' conclusion: anyone who wants flexible ongoing cover is better off with beautyprotect; medassure scores with one-off, precisely planned procedures.

Insuring capsular contracture correctly
Capsular contracture is the most frequent reason why complication insurance is needed: up to 20 % of all women with breast implants develop a hardening requiring treatment (Baker grade 3 or 4) within ten years – the statutory health insurer does not pay for the operation and may even reclaim costs. All the more important, then, to look at the terms and conditions: medassure beauty and safe4beauty insure only grades 3 and 4 and reject customers with existing capsular contracture; beautyprotect PRO/VIP is the only tariff that covers all Baker grades and also insures implants already in place. Frequently asked questions & technical terms explained simply
What is an actuary?
An actuary (Aktuar) is a state-recognised insurance mathematician. Actuaries assess risks, calculate tariffs and review the financial stability of insurance products on a mathematical basis.
What are AVB in complication insurance?
AVB stands for Allgemeine Versicherungsbedingungen, the general terms and conditions. They contain the contractual agreements between the insurer and the policyholder (Versicherungsnehmer), in particular the scope of benefits, exclusions and policyholder duties (Obliegenheiten).
What does GKV recourse (§ 52 SGB V) mean?
Anyone who has statutory health insurance (GKV) and suffers a complication after cosmetic surgery is required by law to contribute to the treatment costs – the health insurer is allowed to reclaim the money from the patient. This reclaim is called recourse (Regress). Complication insurance assumes precisely this claim, with the tariffs compared here up to EUR 300,000.
What does Regelversorgung mean?
Standard statutory care (Regelversorgung) is normal medical care via the statutory health insurer or private health insurance – that is, doctors with statutory health insurance accreditation (Kassenzulassung) and public hospitals. The opposite is private treatment outside standard statutory care, e.g. with a private doctor or in a private clinic, which you have to pay for entirely yourself.
What does "subsidiary" mean?
Subsidiary (subsidiär) means secondary: complication insurance only pays what remains after the statutory health insurer, private health insurance or civil servants' health subsidy (Beihilfe) have paid first. It therefore steps in for the gap, not for the entire invoice.
What are Obliegenheiten?
Policyholder duties (Obliegenheiten) are obligations that the insured person must fulfil in order for the insurance to pay – for example reporting a complication immediately or submitting invoices and documents. Anyone who breaches a policyholder duty risks their claim to benefits.
What is the GOÄ and the 3.5-times rate?
The GOÄ (Gebührenordnung für Ärzte, the scale of fees for doctors) determines how much doctors may charge for private treatments. Depending on the effort involved, the base price may be multiplied by a factor – up to 3.5 times the rate is customary. Some tariffs reimburse private invoices only up to this limit.
What is an Assekuradeur?
A managing general agent (Assekuradeur) is a specialised company that develops, distributes and administers an insurer's products on that insurer's behalf – effectively the link between insurer and customer. The insurance risk itself, however, is borne by the insurer behind it.
What is capsular contracture and what do the Baker grades mean?
Capsular contracture (Kapselfibrose) is the most frequent complication after a breast augmentation: the scar tissue around the implant hardens. The severity is classified in Baker grades from 1 (barely noticeable) to 4 (hard, painful, operation required). The tariffs differ considerably in which grades are insured.
