Cosmetic surgery and health insurance: who pays for what?
The GKV generally does not cover aesthetic procedures – and often not the treatment of complications afterwards either. What really applies and where the insurance gap lies.

Basic rule: statutory health insurers do not pay for aesthetic operations
The statutory health insurance (GKV) covers the costs of medically necessary treatment. A purely aesthetically motivated operation – breast augmentation, facelift, liposuction – is considered not medically necessary and is generally not reimbursed. This also applies to follow-up treatment after complications.
Exceptions: when does health insurance pay after all?
- Breast reconstruction after mastectomy: Following breast cancer surgery, the GKV covers the reconstruction in full.
- Nose correction with demonstrated obstructed breathing: Functional rhinoplasty (septum correction) is eligible for reimbursement.
- Eyelid correction with a restricted field of vision: Where the drooping upper eyelid measurably impairs sight.
- Body lift after extreme weight loss: Recognised in rare cases where symptoms are documented (intertrigo, eczema) – requires extensive documentation.
- Gynaecomastia surgery: Possibly eligible for reimbursement in men with clinically relevant enlargement of the mammary gland.
Private health insurance (PKV): no automatic entitlement
Those insured under the PKV likewise have no automatic entitlement to reimbursement for aesthetic procedures. Most PKV tariffs explicitly exclude purely cosmetic treatment. Check your policy terms and conditions (AVB) or ask your PKV insurer in writing – before the procedure, not afterwards.
The decisive problem: complications after aesthetic surgery
Even if the original operation was paid for privately, the GKV can also refuse to cover the treatment of complications – arguing that the complication results from a self-initiated aesthetic measure. In practice this risk is handled inconsistently, but it is real.
Complication insurance (Folgekostenversicherung) closes precisely this gap: it covers the costs of complications regardless of whether the GKV or the PKV pays.
